The goals of this CME program are to increase awareness among clinicians about the need for comprehensive cancer survivorship care and specific areas where improved survivorship care can lead to improved patient outcomes. This program will educate participants regarding the evolving evidence surrounding the unique medical risks and psychosocial concerns that cancer survivors face from diagnosis through long-term survivorship. The participants will learn to recognize evidence-based screening, prevention and management strategies for potential long-term, late effects and how to tailor these for patients at varying risks for problems. This course will also introduce, highlight and encourage the participant to utilize the emerging body of guidelines, tools, and resources that are available to optimize the coordination and delivery of survivorship care. We have added several modules to this second version of our course to address specific groups and types of survivors that were not covered
