Although these techniques have been proven to be more accurate in the assessment of heart chamber geometry and function and provide additional prognostic value compared with conventional two-dimensional echocardiography (2DE), they are still perceived as being time-consuming.1-4 Recently, new automated on-cart tools have been demonstrated to perform accurate, fast, and reproducible strain and 3DE analyses of the heart chambers.

Echocardiography is frequently the first test to raise the suspicion of transthyretin cardiac amyloidosis (ATTR-CM). A “granular sparkling” appearance of the thickened ventricular myocardium is a characteristic echocardiographic feature of cardiac amyloidosis.1 This was first reported in the 1980s, based on ultrasound imaging using fundamental frequencies. Harmonic imaging, the current technique, attenuates the “sparkling” appearance, making it challenging to detect by the human eye. A contemporary study showed the sparkling appearance is an unreliable visual marker of disease; it is present in 25% of patients with cardiac amyloidosis and 12.5% of those with hypertrophic cardiomyopathy on visual assessment.

Left ventricular outflow tract obstruction, occurring at rest or with provocation in 75% of HCM patients, portends adverse prognosis, contributes to symptoms, and is frequently a therapeutic target.

Although valve repair is frequently performed, especially for mitral and tricuspid regurgitation, valve replacement remains common, particularly in adults. Diagnostic methods are often needed to assess the function of the prosthesis. Echocardiography is the first-line method for noninvasive evaluation of prosthetic valve function. The transthoracic approach is complemented with two-dimensional and three-dimensional transesophageal echocardiography for further refinement of valve morphology and function when needed.

Screening and detection strategies in younger children have largely relied on longitudinal monitoring of echocardiographic measures of left ventricle (LV) systolic function, mainly fractional shortening (FS) and ejection fraction (EF). Despite the important role of echocardiography in the follow-up of this patient group, a recent multicenter study indicated that the overall quality of routine pediatric echocardiograms for childhood cancer survivors was not sufficient to reliably assess LV function.
