Mashup Score: 32Arthroscopic Patella Realignment for Children And Adolescents: A Single Incision Technique - 24 hour(s) ago
Management of patellofemoral conditions in children and adolescents can be extremely challenging. Apart from the patellofemoral instability patients, there are several other disorders that can cause significant morbidity, patellofemoral maltracking, patellar tilt, and Hoffa’s fat pad impingement syndrome are common problems that may not respond to nonoperative management. Understanding the exact pathology and apply the appropriate management is critical for successful outcome. Identification of novel surgical interventions that can provide symptom relief in a safe manner is important.

Mashup Score: 10Modified Single–Working Portal Technique Using Percutaneous Spinal Needle Suture Passing in Arthroscopic Subscapularis Repair - 2 day(s) ago
The main method for arthroscopic repair of the subscapularis is repair with suture anchors. The surgeon generally establishes the anterior and anterolateral operation portals to complete anchor implantation and suture passing, respectively. The single–operation portal technique has been developed recently. However, in the traditional single–operation portal technique, the suture device and grasper are difficult to operate simultaneously. In addition, with the traditional rotator cuff suture device, it is easy to cause further iatrogenic injury to the rotator cuff because of its larger diameter.

Mashup Score: 18The Subscapularis-Sparing “Flipped Latarjet” Procedure - 3 day(s) ago
The Latarjet procedure is a proven and effective operation to treat anterior shoulder instability. Especially in cases with anterior glenoid bone loss, the Latarjet operation is the most popular procedure to restore glenoid anatomy and avoid further dislocations. Next to the re-creation of the missing glenoid bone, the sling effect of the conjoint tendon transferred between a split in the subscapularis muscle is an important “soft tissue stabilizer” of the humeral head. However, it has been shown that the inferior part of the subscapularis muscle tends to degenerate, leading to fatty infiltration of the muscle itself.

Mashup Score: 92Circumferential Cross-Stitch Suture for Longitudinal Lateral Meniscal Tear Around the Popliteal Tendon Hiatus Region - 6 day(s) ago
Lateral meniscal repair of the popliteal hiatus is technically demanding. The inside-out technique requires an additional incision and carries the risk of posterolateral soft tissue damage to the knee joint. In addition, the presence of the popliteal tendon limits the route of the suture thread. Within the current trend of the all-inside suture technique, meniscal suture-based all-inside repair demonstrates biomechanical advantages over anchor-based all-inside repair. We introduce a meniscal suture-based all-inside meniscal repair technique for longitudinal lateral meniscal tears.

Mashup Score: 28Inside-Out Anchor Placement: A Technique to Instrument Far Medial Anchors in Acetabular Labral Repair - 7 day(s) ago
Labral tears most commonly occur anteriorly between the 12- and 3-o’clock positions, with the 12-o’clock position denoted as superior and the 3-o’clock position denoted as anterior. When approaching the 3-o’clock position and beyond, suture anchor placement becomes difficult given the challenging arthroscopic trajectory and an overall thin anterior rim of cortical bone for anchor purchase, resulting in a narrow angle of safe drilling. The purpose of this technical note is to present a safe and reproducible method of suture anchor placement during acetabular labral repair approaching the 3- and 4-o’clock positions, with the 12-o’clock position representing the superior anatomic location and the 3-o’clock position representing the anterior anatomic location regardless of hip laterality.

Mashup Score: 12Arthroscopic Confirmation of Femoral Button Deployment During Posterior Cruciate Ligament Reconstruction - 8 day(s) ago
Posterior cruciate ligament (PCL) ruptures are uncommonly seen in knee ligament injuries. Cconservative treatment is often suitable for isolated tears with mild-to-moderate posterior knee laxity (grades I or II). However, surgical intervention is indicated for symptomatic grade III or multiligament knee injuries. PCL reconstruction has experienced continuous development due to the progress made in arthroscopic techniques and instruments. Abnormal positioning and tensioning of the femoral button result in multiple complications such as residual laxity, loss of quadriceps muscle strength, and joint stiffness.

Mashup Score: 45Arthroscopic Lateral Retinacular-Lengthening Procedure - 8 day(s) ago
Anterior knee pain is a common musculoskeletal complaint that is often due to an excessively tight lateral retinaculum, which normally plays a role in patellar tracking and stabilization. Several etiologies underlie lateral soft-tissue tightness in the knee, including lateral patellar compression syndrome, patellofemoral arthritis, patellofemoral instability, and patellofemoral pain syndrome. Stretching the lateral retinaculum through conservative treatment may be helpful, but lateral retinacular lengthening may be indicated.

Mashup Score: 25Patient Positioning for Postless Hip Arthroscopy - 10 day(s) ago
Hip arthroscopy has been on the rise since its inception. Initial descriptions of the procedure required skeletal traction of the operative extremity with countertraction provided by a perineal post. Perineal posts are associated with complications such as perineal nerve palsy, genital laceration, and hematoma formation. Postless traction has been developed to avoid complications related to use of perineal posts. A patient is positioned supine on a postless traction table (Guardian; Stryker, Greenwood Village, CO) with a disposable traction pad.

Intraosseous injections of bone marrow aspirate concentrate have shown promise in the treatment of bone marrow lesions (BMLs) in the knee. With the wide-awake limited anesthesia no tourniquet (WALANT) technique, intraosseous injections can be performed with the patient under local anesthesia in the procedure room or operating room setting. This article describes 2 techniques to access the BML of interest. The “decompression route” involves drilling through the nearest cortex, and the “biologic route” involves drilling through healthy bone to promote bleeding and the introduction of healthy biologic tissue to the BML.
