I have seen many cases of dementia during my career. Throughout my internship and residency, the hospital was never short of patients at various stages of cognitive decline—sometimes sitting in bed mildly confused, other times agitated and restless in the hallways. As a training neurologist, I studied and observed the spectrum of dementia subtypes and presentations, performing cognitive tes ts, formulating lists of differentials for patients with memory disorders, and counselling families on the diagnosis.

The patient noted a painful muscle rippling after accidentally dropping a heavy weight on his quadriceps. Upon presentation, he had full strength throughout. Thenar percussion myotonia was negative. EMG showed myotonic discharges, and genetic testing confirmed one pathogenic variant of th e CNBP gene with > 75 repeats consistent with a diagnosis of myotonic dystrophy type 2 (DM2).
