Stage E Cardiogenic Shock: Hold 'Em, Fold 'Em, or Double Down? - 2 month(s) ago

Cardiogenic shock (CS) remains a leading cause of mortality in patients with acute myocardial infarction (AMI), and its incidence is increasing.1 While mortality has exceeded 50% for decades, contemporary registries and the recent randomized DanGer Shock trial have renewed hopes of breaking the glass ceiling of 50% survival.2–4 Refinements to (and increasing uniformity of) shock staging—most notably using the Society for Cardiovascular Angiography & Interventions (SCAI) staging system—have helped differentiate etiologies, phenotypes, and stages of CS (previously lumped together under a single “shock” umbrella), permitting better “apples-to-apples” comparisons of patients and management strategies.
