Mashup Score: 1Factors associated with insulin resistance in a... : AIDS - 22 day(s) ago
ar disease (CVD) risk factors. Participants 10–18 years of age were included in a prospective study performed in Kampala, Uganda. We compared baseline and changes in insulin resistance (by HOMA-IR) and in markers of inflammation at baseline and 96 weeks. PHIVs were on ART with HIV-1 RNA level 400 copies/ml or less. Generalized Estimating Equation models were used to assess associations between HOMA-IR, and demographic as well as inflammatory markers. Of the 197 participants recruited at baseline (101 PHIV, 96 HIV-negative), 168 (89 PHIV, 79 HIV-negative) had measurements at 96 weeks. At baseline, median (Q1, Q3) age was 13 years (11,15), 53.5% were women, median CD4+ cell counts were 988 cells/μl (631, 1310). At baseline, HOMA-IR was significantly higher in PHIV than in controls (P = 0.03). HOMA-IR did not significantly change by week 96 in either group, and at 96 weeks, was similar between groups (P = 0.15). HOMA-IR was not associated with any inflammatory markers, or any specific ART

Mashup Score: 0Effect of HIV stigma on depressive symptoms, treatment... : AIDS - 22 day(s) ago
sing socio-demographics, antiretroviral therapy (ART) adherence, depressive symptoms (PHQ-9), and HIV stigma (10-item Wright scale). First viral load (VL) after enrollment was abstracted from records. We estimated risk of depressive symptoms (score>4), non-adherence (missing ≥2 days of ART in a month), and detectable VL (≥50 copies/ml) for each standard deviation (SD) increase in HIV stigma score, adjusted for age and gender (and regimen in VL model). The generalizing estimating equation models included measures for the 3 visits. Results: Median age for the 1,011 YWHIV was 18 years. At baseline, frequency of non-adherence, depressive symptoms and detectable VL was 21%, 21% and 46%, respectively. Mean stigma score was 25 (SD = 7.0). Each SD stigma score increment was associated with higher risk of depressive symptoms (adjusted relative risk [aRR] 1.31 [95% confidence interval [CI]: 1.20–1.44]), non-adherence (aRR 1.16 [CI: 1.05–1.27]) and detectable VL (aRR 1.20 [CI: 1.08–1.32]). cebo, and behavioral cessation therapy, positively smoke free (PSF), compared with standard of care (SOC) among PWH who smoke. The study was designed with power to detect a small effect (Cohen’s h of 0.28–0.36) with 240 participants. The primary outcome was the 7-day point prevalence abstinence (PPA) confirmed by exhaled carbon monoxide (ECO) less than 10 ppm for both main effects at 36 weeks. The study was conducted from June 2016 to November 2020. During the study’s last year, recruitment was halted because of COVID-19. Results: The study randomized 184 participants with power to detect a medium effect (Cohen’s h of 0.41). Participants were mostly African American (89.7%), men (62.8%) who smoked mentholated cigarettes (96.7%). Nearly all received antiretroviral medication (96.2%). Quit rates for the entire sample were 7.5% at 36 weeks. Compared with those who received placebo, neither those who received varenicline [36 weeks; OR (95% CI), 1.31 (0.33–5.22), P = 0.70] nor PSF [36 weeks

There is an urgent need to address the high rates of smoking amongst PWH. This trial tested two proven interventions to support smoking cessation but could not show any benefit of either, suggesting new strategies are needed to help PWH to stop smoking. 🔗https://t.co/JpanfONtnn https://t.co/IbasZjHoHl

Mashup Score: 0Association of age at antiretroviral therapy initiation... : AIDS - 1 month(s) ago
y of people attending an HIV clinic at the Royal Free Hospital NHS Trust, London, who initiated ART between 2001 and 2015, and achieved and maintained HIV-1 viral suppression (viral load <1,000 copies/ml). The association of age group at ART initiation with CD4+ : CD8+ ratio at 5 and 10 years was assessed. Methods: Multivariable linear regression was used to investigate the relationship between age at ART initiation and log CD4+ : CD8+ ratio, adjusting for demographic factors (gender/HIV transmission route, ethnicity), baseline CD4+ count and calendar year. Results: The sample included 1859 people aged 20–78 (75% men, 56% white ethnicity). Overall, median CD4+ : CD8+ T-cell ratio increased from 0.24 at baseline to 0.77 at year 5 and 0.88 at year 10. Ratios increased among all age groups in unadjusted and adjusted models but increased less among older ages (baseline ages 60–69 and 70–79). Median ratios at year 5 were 0.85, 0.80, 0.72, 0.76, 0.6, and 0.44, respectively, among people aged

Mashup Score: 0The clinical utility of three frailty measures in... : AIDS - 1 month(s) ago
ers. Methods: The study involved 284 people with HIV (PWH) at least 50 years enrolled at UC San Diego’s HIV Neurobehavioral Research Program. Frailty measurements included the Fried Phenotype, the Rockwood Frailty Index, and the Veterans Aging Cohort Study (VACS) Index. HAND was diagnosed according to Frascati criteria. ANOVAs examined differences in frailty severity across HAND conditions. ROC analyses evaluated sensitivity and specificity of each measure to detect symptomatic HAND [mild neurocognitive disorder (MND) and HIV-associated dementia (HAD)] from no HAND. Results: Across all frailty measures, frailty was found to be higher in HAD compared with no HAND. For Fried and Rockwood (not VACS), frailty was significantly more severe in MND vs. no HAND and in HAD vs. ANI (asymptomatic neurocognitive impairment). For discriminating symptomatic HAND from no HAND, Fried was 37% sensitive and 92% specific, Rockwood was 85% sensitive and 43% specific, and VACS was 58% sensitive and 65% spe

Mashup Score: 0Ambient particulate matter and renal function decline in... : AIDS - 1 month(s) ago
lationship between 1 and 28 day moving averages of particulate matter concentrations with Scr and eGFR was assessed using linear mixed-effects models. Modified Poisson regression models were employed to assess the associations of cumulative particulate matter exposure with the incidence of chronic kidney disease (CKD). Mediation analyses were used to examine the role of TyG index. Results: Short-term exposure to particulate matter was related to reduced renal function. The strongest associations between exposure to particulate matter (PM)1, PM2.5, and PM10 and percentage changes in eGFR were observed at 7-day moving average exposure windows, with a respective decrease of 0.697% (−1.008%, −0.386%), 0.429% (−0.637%, −0.220%), and 0.373% (−0.581%, −0.164%) per IQR increment. Long-term exposure to PM1, PM2.5, and PM10 was positively linked with the incidence of CKD, with each IQR increment corresponding to fully adjusted RRs (95% CIs) of 1.631 (1.446–1.839), 1.599 (1.431–1.787), and 1.903

rch Network of Integrated Clinical Systems (CNICS) cohort. Methods: We identified adult PWH who initiated semaglutide between 2018 and 2022 and with at least two weight measurements. The primary outcome was within-person bodyweight change in kilograms at 1 year. The secondary outcome was within-person Hemoglobin A1c percentage (HbA1c) change. Both outcomes were estimated using multivariable linear mixed model. Results: In total, 222 new users of semaglutide met inclusion criteria. Mean follow-up was 1.1 years. Approximately 75% of new semaglutide users were men, and at baseline, mean age was 53 years [standard deviation (SD): 10], average weight was 108 kg (SD: 23), mean BMI was 35.5 kg/m2, mean HbA1c was 7.7% and 77% had clinically recognized diabetes. At baseline, 97% were on ART and 89% were virally suppressed (viral load < 50 copies/ml). In the adjusted mixed model analysis, treatment with semaglutide was associated with an average weight loss of 6.47 kg at 1 year (95% CI −7.67 to
