On January 7, 2024, our cardiac electrophysiology community lost Bruce L. Wilkoff, MD, one of the most impactful and pioneering members of the Heart Rhythm Society (HRS). Dr Wilkoff was an innovator in the field of cardiac implantable electronic devices (CIEDs). Recognizing the impact of these devices on preventing death in our patients, he sought to reduce the morbidity and complications associated with arrhythmia therapies, particularly advancing the areas of device programming, rate response, antitachycardia pacing, implantation and extraction techniques, lead management, management of device recalls, and new technologies.

Mashup Score: 26Repeat Catheter Ablation for Atrial Fibrillation - 4 month(s) ago
Catheter ablation of atrial fibrillation (AF) is an established therapy that reduces AF burden, improves quality of life, and reduces the risks of cardiovascular outcomes. While there are clear guidelines for the application of de novo catheter ablation, there is less evidence to guide recommendations for repeat catheter ablation in patients who experience recurrent AF. In this review, we examine the rationale for repeat ablation, mechanisms of recurrence, patient selection, optimal timing, and procedural strategies.

Atrial fibrillation has a multifactorial pathophysiology influenced by cardiac autonomic innervation. Both sympathetic and parasympathetic influences are profibrillatory. Innovative therapies targeting the neurocardiac axis include catheter ablation or pharmacologic suppression of ganglionated plexi, renal sympathetic denervation, low-level vagal stimulation, and stellate ganglion blockade.
