Sudden cardiac death in children and young adults is a relatively rare but tragic event whose pathophysiology is unknown at the molecular level. Evidence indicates that the main cardiac sodium channel (NaV1.5) and the strong inward rectifier potassium channel (Kir2.1) physically interact and form macromolecular complexes (channelosomes) with common partners, including adapter, scaffolding, and regulatory proteins that help them traffic together to their eventual membrane microdomains. Most important, dysfunction of either or both ion channels has direct links to hereditary human diseases.

Infection is a major complication of cardiac implantable electronic device (CIED) implantation. A recent Cochrane systematic review of barrier dressings to prevent infections during non-CIED related surgery showed no significant difference regarding infection with use of iodine-impregnated dressings and an increased incidence with non-impregnated dressings. Based on these non-CIED studies, current guidelines do not recommend barrier dressings for CIED surgery.

Older clinical trials prospectively confirmed ATP as a painless method to terminate fast ventricular tachycardias (FVT), but these trials predominantly included secondary prevention ICD recipients and relatively short times from detection to intervention. Premature intervention likely resulted in the overestimation of reported ATP success on what otherwise may have been self-terminating events. Emergence of novel programming guidelines to reduce premature and inappropriate therapies and the availability of ICD technologies lacking ATP capabilities, requires the re-evaluation of the efficacy of initial ATP in terminating FVT in primary prevention (PP) cohorts.
