A 44-year-old man presented with decreased vision for 1 week (best-corrected visual acuity: 20/160 right eye, 20/100 left eye). Diagnosis revealed antiphospholipid syndrome and thrombotic microangiopathy featuring positive anti-β2-glycoprotein I of immunoglobulin M, microangiopathic hemolytic anemia (Hb: 59 g/dl), thrombocytopenia (platelet count: 30 × 109/L), and acute kidney failure. Fundus photography showed Purtscher flecken (A, blue arrowhead) and intraretinal hemorrhages. Ultrawidefield OCT angiography manifested extensive retinal nonperfusion area (B).

Autofluorescence (not shown) showed hypoautofluorescence and hyperautofluorescence spots with no clear pattern. Fluorescein angiography (C) displayed a distinctive butterfly-shaped pattern of hypofluorescent radiating lines.

A 33-year-old man presented with decreased vision in the right eye 2 months after trauma. Fundus examination revealed retinal hemorrhage along a choroidal rupture (A). OCT demonstrated a hyperreflective membrane extending from the choroid into the subhyaloid space (B). It is likely there was a full thickness retinal break along with a choroidal rupture at the time of the trauma. The epiretinal membrane is likely a result of the previous vitreous hemorrhage and communicates with the subretinal fibrosis from the choroidal neovascular membrane due to the retinal break.
