Caregivers of patients with chronic kidney disease from rural communities play a crucial role in access to dialysis and transplantation, but they face many challenges including geographical distance, financial hardship, and limited support. This study aimed to inform strategies to overcome these challenges by describing the experiences of caregivers of patients with kidney failure from rural Australian communities in accessing kidney replacement therapy.

The difference between cystatin C-based and creatinine-based estimated glomerular filtration rate (eGFRdiff) has been suggested to reflect factors distinct from kidney function that are associated with cardiovascular risk. However, the association between eGFRdiff and atrial fibrillation (AF) risk has not been extensively evaluated.

Mashup Score: 5Development and Validation of the Rating of CKD Knowledge Among Older Adults (Know-CKD) with Kidney Failure - 2 day(s) ago
Few older adults with kidney failure engage in shared decision-making (SDM) for kidney replacement therapy. Absence of instruments assessing SDM-relevant knowledge domains may contribute to this. We assessed the reliability and validity of a new instrument, the Rating of CKD Knowledge Older Adults (Know-CKD).

Mashup Score: 14Infections Following Kidney Transplantation After Exposure to Immunosuppression for Treatment of Glomerulonephritis - 4 day(s) ago
Many kidney transplant patients have glomerular disease as the cause of kidney failure. These patients may be exposed to immunosuppression before transplantation which could increase the risk for infections following receipt of a transplanted kidney. We identified kidney transplant recipients at a university teaching hospital who received immunosuppression pre-transplant for the treatment of glomerular kidney disease. We examined their risk for infection after transplantation by comparing it to the risk among transplant patients who were not exposed to immunosuppression prior to transplant. We observed no increased risk for infection following exposure to prior immunosuppression. Therefore, patients exposed to significant amounts of immunosuppression before transplantation may not require special surveillance or medication adjustment for fear of infection following their receipt of a kidney transplant.

Mashup Score: 9Associations of Lack of Insurance and Other Sociodemographic Traits With Follow-up After Living Kidney Donation - 4 day(s) ago
Clinical and laboratory follow-up for living kidney donors (LKDs) may quantify risk, prompt early intervention, and inform donor candidates and donors.1 In 1999, the Organ Procurement and Transplantation Network (OPTN) requested that US transplant centers submit LKD follow-up forms 6 and 12 months after donation.2 Additional elements such as insurance status were added in 2004, with follow-up extended to 24 months in 2008. Noting poor performance, in 2013, the OPTN defined minimum transplant center follow-up thresholds, targeted to ≥80% and ≥70% for clinical and laboratory data, respectively.

Individuals with a low estimated glomerular filtration rate (eGFR) are at a high risk of death. Yet, the causes underpinning this association are largely uncertain. This study aimed to assess the causal relationship of low eGFR with all-cause and cause-specific mortality.

Mashup Score: 41A Holistic Framework for the Evaluation of Kidney Function in a Gender-Diverse Landscape - 5 day(s) ago
The most commonly used equations to estimate glomerular filtration rate incorporate a binary male-female sex coefficient, which has important implications for the care of transgender, gender-diverse, and nonbinary (TGD) people. Whether “sex assigned at birth“ or a binary “gender identity“ is most appropriate for the computation of estimated glomerular filtration rate (eGFR) is unknown. Furthermore, the use of gender-affirming hormone therapy (GAHT) for the development of physical changes to align TGD people with their affirmed gender is increasingly common, and may result in changes in serum creatinine and cystatin C, the biomarkers commonly used to estimate glomerular filtration rate.
