Mashup Score: 7Pericardial Disease in Cardiac Amyloidosis - 3 day(s) ago
In patients with cardiac amyloidosis, pericardial involvement is common, with up to half of patients presenting with pericardial effusions. The pathophysiological mechanisms of pericardial pathology in cardiac amyloidosis include chronic elevations in right-sided filling pressures, myocardial and pericardial inflammation due to cytotoxic effects of amyloid deposits, as well as renal involvement with subsequent uremia and hypoalbuminemia. The pericardial effusions are typically small; however, several cases of life-threatening cardiac tamponade with hemorrhagic effusions have been described as a presenting clinical scenario.

Mashup Score: 4Metabolic acidosis in anaesthesia and critical care - 12 day(s) ago
Metabolic acidosis is common in acutely unwell patients and indicates an underlying pathophysiological disturbance. Treatment should be primarily directed at the underlying condition, not the acidosis. Thus, a key element of managing metabolic acidosis is determining the underlying cause. In most cases, metabolic acidosis is caused by increased plasma concentrations of pathologic anions and results in an increased anion gap. However, metabolic acidosis can occur in the presence of a normal or low anion gap.

Mashup Score: 7Perioperative care of patients undergoing total hip arthroplasty - 12 day(s) ago
Around 80,000 total hip arthroplasties (THA) are performed in England each year with this number expected to increase to 96,000 by 2035.1 Currently more than half of patients are aged >70 yrs. In the USA it is projected around 1.4 million THA will be performed annually by 2040. Given these numbers, even small improvements in care at an individual level may translate into significant benefits at a population level.

Mashup Score: 6B-cell non-Hodgkin lymphomas - 17 day(s) ago
B-cell lymphomas occur with an incidence of 20 new cases per 100 000 people per year in high-income countries. They can affect any organ and are characterised by heterogeneous clinical presentations and courses, varying from asymptomatic, to indolent, to very aggressive cases. Since the topic of B-cell non-Hodgkin lymphomas was last reviewed in The Lancet in 2017, a deeper understanding of the biological background of this heterogeneous group of malignancies, the availability of new diagnostic methods, and the development and implementation of new targeted and immunotherapeutic approaches have improved our ability to treat patients.
