On this episode of JHLT: The Podcast, the JHLT Digital Media Editors explore two studies from the May issue of The Journal of Heart and Lung Transplantation. Digital Media Editor David Schibilsky, MD, a cardiothoracic surgeon from Freiburg, Germany, hosts this episode. First, Dr. Schibilsky and Digital Media Editor Erika Lease, MD, interview their first guest, Helen A. Hannan, the podcast’s first-ever undergraduate pre-medical guest, from the University of Michigan. Helen was the lead author on the study “.” The study noted that prior research in utilization of kidneys from donors with Hepatitis C had shown disparities due to gender and education—and wanted to see if this was the case in heart and lung donors as well. Drs. Schibilsky and Lease chat with Helen about the findings of the study, including some of the interesting differences shown between heart and lung recipients, and the ramifications for better patient discussions at the clinical level. Next, Dr. Schibilsky and Dig

Adult T-cell leukemia/lymphoma (ATLL) is a rare disease caused by the human T-lymphotropic virus (HTLV). Only 4% of patients infected with HTLV contract ATLL, and most take decades to present with malignancy. Herein, we present a rare case of ATLL following a heart transplantation.

To verify whether the new hemodynamic definition of pulmonary hypertension (PH) has any implication in treatment of Chronic Thrombo-Embolic Pulmonary Disease (CTEPD) patients without PH, we retrospectively analysed the clinical and functional changes determined by pulmonary endarterectomy (PEA) in 63 CTEPD patients without PH who underwent surgery at our center, comparing those in whom the hemodynamic diagnosis of PH met recent guideline recommendations versus those in whom the diagnosis only met previous hemodynamic thresholds.

Mashup Score: 7Blockade of BLyS inhibits B-cell responses and antibody production for the prevention of chronic transplant rejection - 8 day(s) ago
Chronic rejection, closely related to the activation of B cells and donor-specific antibody (DSA) production, has unsatisfactory therapeutic outcomes. B lymphocyte stimulator (BLyS) is a major regulatory factor that controls the activation and differentiation of B cells. However, it remains unclear whether BLyS blockade can regulate B and plasma cells in the transplantation setting and affect chronic rejection. Here, we investigated the efficacy of the BLyS inhibitors belimumab and telitacicept in controlling B-cell response and preventing chronic rejection.

The physiological response of the right ventricle (RV) following left ventricular assist device (LVAD) implantation is difficult to predict. We aimed to investigate RV geometric and functional changes after LVAD insertion and their effects on clinical outcomes.

Changes in RV dimensions are not included in risk predictions post LVAD. A non-adapted RV was associated with alterations TV & RV geometry, leading to worsening TR. This may help identify patients that may be refractory to LVAD therapy. @HideyukiHayash6 🔗 https://t.co/m0vkYBMX3T https://t.co/cB6Y6O1xjn

Mashup Score: 5Development and validation of primary graft dysfunction predictive algorithm for lung transplant candidates - 13 day(s) ago
Primary graft dysfunction (PGD) is the leading cause of early morbidity and mortality after lung transplantation. Accurate prediction of PGD risk could inform donor approaches and perioperative care planning. We sought to develop a clinically useful, generalizable PGD prediction model to aid in transplant decision-making.

The Heartmate 3 (HM3) risk score (HM3RS) was derived and validated internally from within the Multicenter Study of MagLev Technology in Patients Undergoing Mechanical Circulatory Support Therapy with HeartMate 3 (MOMENTUM 3) trial population and provides 1- and 2-year mortality risk prediction for patients in those before HM3 left ventricular assist device (LVAD) implantation. We aimed to evaluate the HM3RS in nontrial unselected patients, including those not meeting inclusion criteria for MOMENTUM 3 trial enrollment.

Mashup Score: 1Impact of recipient age on mortality among Cytomegalovirus (CMV)-seronegative lung transplant recipients with CMV-seropositive donors - 15 day(s) ago
Cytomegalovirus (CMV)-seronegative lung transplant recipients (LTRs) with seropositive donors (CMV D+/R−) have the highest mortality of all CMV serostatuses. Due to immunosenescence and other factors, we hypothesized CMV D+/R− status might disproportionately impact older LTRs. Thus, we investigated whether recipient age modified the relationship between donor CMV status and mortality among CMV-seronegative LTRs.
