Prematurity is the leading cause of death in newborns and children younger than 5 years both globally and in sub-Saharan Africa.1 Kangaroo mother care (KMC) is recommended as an effective intervention to reduce this mortality.2–4 Originally defined as skin-to-skin contact between a mother and her newborn, frequent and exclusive or nearly exclusive breastfeeding, and early discharge from hospital, KMC has been proposed as an alternative to conventional neonatal care for low birthweight babies. Large randomised controlled trials, including the WHO iKMC trial5 and the OMWaNA trial published in this issue of The Lancet,6 provide varying evidence of the effect of KMC initiated before clinical stabilisation of the newborn on reducing the rate of neonatal mortality.

A 30-month-old girl attended our clinic for exploration of optic nerve atrophy after she initially presented with respiratory difficulties at 20 months of age. The patient, who had been born at 39 weeks of gestation, also had feeding difficulties, nasal obstruction, and a reduced ability to open her mouth; she had developed progressive macrocephaly in the first few months of life (appendix).

This week, The Lancet published the findings from COMMEND, the largest clinical trial to date of a psychological intervention for people with motor neuron disease (MND). This devastating, incurable neurodegenerative disease results from the loss of motor neurons in the motor cortex and spinal cord, causing progressive paralysis and eventual death. More than half of people with MND die within 2 years of a diagnosis. Rates of depression and anxiety in patients are high. Given the dire prognosis, interventions that can improve the quality of remaining life for patients are vital.
