A 47-year-old woman came to our institution with a 1-month history of breast erythema, swelling, and a new 1 cm purple skin plaque on her left breast which rapidly increased in size over the following weeks. She had no history of breast trauma. 6 years earlier, she had been treated for luminal A invasive ductal carcinoma in this breast with neoadjuvant chemotherapy followed by breast-conserving surgery, adjuvant radiation therapy, and oral tamoxifen. Mashup Score: 24Vasculitis: the promising road ahead - 1 day(s) ago
Vasculitides have long posed significant diagnostic and therapeutic challenges for clinicians due to their complex nature. During the past two decades, the mechanisms driving these conditions have become better understood, offering hope to many patients. Yet, the clinical management of patients with vasculitis remains challenging and treatment options are limited. Where are we on the road to combating these diseases?

Telerehabilitation with a physiotherapist was non-inferior to in-person care for chronic knee pain in a randomised non-inferiority trial by Rana Hinman and colleagues. 394 adults with chronic knee pain consistent with osteoarthritis were randomly assigned to receive five in-person (n=204) or telerehabilitation (n=190) physiotherapist consultations. At 3 months, both groups had improvements in pain (co-primary outcome; mean change 2·98 [SD 2·23] for in-person care and 3·14 [1·87] for telerehabilitation; mean difference 0·16 [95% CI –0·26 to 0·57]).

Mashup Score: 13Cardiovascular risk in young people with childhood onset systemic lupus erythematosus - 6 day(s) ago
Since 1940, epidemiologists, clinicians, and scientists promoted increased awareness of the crucial importance of promoting preventive strategies for minimising the risk of poor health later in life. This is particularly relevant to children and young people with chronic inflammatory diseases, including childhood-onset systemic lupus erythematosus (SLE), which are almost always associated with increased risk of comorbidity. A wealth of literature emphasises the contribution of chronic inflammation to the development of subclinical atherosclerosis, which drives the increased risk of cardiovascular disease, and many guidelines across specialties highlight the need for cardiovascular disease risk stratification and tailored management.

Two conventional synthetic DMARD tapering strategies were associated with significantly lower rates of flare-free survival compared with stable conventional synthetic DMARD treatment, and the data do not support non-inferiority. However, drug-free remission was achiveable for a significant subgroup of patients. This trial provides information on risk and benefits of different treatment strategies important for shared decision making.

Little is known about the natural course and long-term outcomes of childhood-onset Sjögren’s disease due to its low prevalence, difference in clinical presentation between children and adults, and paucity of good quality research in paediatric populations. Delays in recognition of childhood-onset Sjögren’s disease are potentially explained by the heterogeneity of clinical manifestations at disease onset1–3 and the poor performance of adult classification criteria in children.4,5 Increased awareness in health professionals and involvement of multi-specialties are needed to improve disease recognition.
