A 66-year-old man without prior history of malignancy was evaluated for a firm, pink nodule on his left bulbar conjunctiva present for > 1 year. A, The lesion was 5×8 mm and neither fixed to the sclera nor involved the cornea. B, Excisional biopsy revealed bland, spindle-cells within a dense collagen matrix of the substantia propria, multiple dilated branching vessels, and absent myxoid elements. The spindle-cells were vimentin/CD34-positive and SOX10/STAT6-negative (C, brown indicates positive).

Metastatic renal clear cell carcinoma mimics eyelid sebaceous carcinoma (SC). A 63-year-old man presented with a solitary mass with large vessels in lower eyelid (A-B). After tumor resection, pathology disclosed vacuolated tumor cells with atypical nuclei suggestive of SC (C, upper part). His eyelid was tumor free after surgery. Nevertheless, he noticed a nasal mass 9 months postoperatively, and subsequent examinations revealed a renal mass. Further biopsy was pathologically proved renal clear cell carcinoma (C, lower part).
