A 43-year-old man had a crater-shaped accessory iris adherent to the collarette of the normal iris in both eyes (A, B). His best corrected visual acuity was 6/6 (+1.50 diopter sphere) in both eyes with normal intraocular pressure. Gonioscopy showed an open angle with pigmented trabecular meshwork in both eyes. Both corneas were smaller, with a 9.2-mm diameter. After dilatation, the normal iris with its pupil disappeared (i.e., was hidden), and the pupil of the accessory iris became more round and taut (C, D) because of the circumferential pull of the normal iris.

Stevens–Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) often leads to substantial ocular surface damage.1-4 Systemic prednisolone has been previously described as a treatment for acute SJS/TEN with variable results; however, a recent study suggests that etanercept is a safe and more effective treatment with superior skin healing and lower mortality rate (8.3% vs 16.3%) when compared to treatment with prednisolone.5 Therefore, etanercept treatment may also reduce ocular complications in SJS/TEN.

Orbit magnetic resonance imaging (MRI) revealed left optic perineuritis with left inferior and medial rectus edema without any evidence of midbrain involvement either of the third nerve nuclei or third nerve bundles within the midbrain (A).
