Mashup Score: 4Cystic Degeneration of Duodenal Gastrointestinal Stromal Tumor Mimicking Pancreatic Solid Pseudopapillary Neoplasm on Imaging - 3 month(s) ago
A 36-year-old woman presented with progressive abdominal fullness and early satiety over 2 months. She had no significant medical, surgical, or family history. On exam, there was a sizable right upper quadrant mass, which was mildly tender to palpation. Abdominal computed tomography showed a 14 × 11 cm cystic mass in the right upper quadrant, suspicious in this clinical context for a solid pseudopapillary neoplasm arising from the pancreatic uncinate process (Figure A). Endoscopic ultrasound showed a large hypoechoic cyst with internal septations (Figure B).

Mashup Score: 1It Is Time for a Simplified Approach to Hepatitis B Elimination - 4 month(s) ago
Hepatitis B virus (HBV) infection continues to threaten millions of lives across the globe, despite universal vaccination efforts. Current guidelines for screening, vaccination, and treatment are complex and have left too many people undiagnosed or improperly managed. Antiviral therapy has been shown to significantly reduce the incidence of liver-related complications, including liver cancer. However, the complexity of existing guidelines can make it difficult to identify which patients to target for treatment, and recommendations that are difficult to implement in real-world settings pose a barrier to eligible patients to receive therapy and contribute to health disparities in HBV care.

A 65-year-old man who had a local recurrence in the oral cavity during follow-up after chemoradiotherapy for oropharyngeal carcinoma underwent percutaneous endoscopic gastrostomy (PEG) because of difficulty in oral intake. He had no underlying disease and no relevant family history and was on no medications including antithrombotic drugs. One month after PEG, the patient developed hematemesis at night. He was in shock, and after his condition was stabilized, emergency esophagogastroduodenoscopy (EGD) was performed.

Mashup Score: 3Obese Patients With Nonalcoholic Fatty Liver Disease Have an Increase in Soluble Plasma CD163 and a Concurrent Decrease in Hepatic Expression of CD163 - 9 month(s) ago
Macrophages play an important role in the development of nonalcoholic fatty liver disease (NAFLD) and its progression to nonalcoholic steatohepatitis (NASH). In this study, we investigated the hepatic expression of the macrophage scavenger receptor CD163 and the plasma level of its shed soluble form (sCD163) in patients with obesity and NASH, non-NASH NAFLD (NAFL), or healthy livers (no NAFLD).
