Echocardiography is frequently the first test to raise the suspicion of transthyretin cardiac amyloidosis (ATTR-CM). A “granular sparkling” appearance of the thickened ventricular myocardium is a characteristic echocardiographic feature of cardiac amyloidosis.1 This was first reported in the 1980s, based on ultrasound imaging using fundamental frequencies. Harmonic imaging, the current technique, attenuates the “sparkling” appearance, making it challenging to detect by the human eye. A contemporary study showed the sparkling appearance is an unreliable visual marker of disease; it is present in 25% of patients with cardiac amyloidosis and 12.5% of those with hypertrophic cardiomyopathy on visual assessment.
