People with diabetes who are receiving dialysis for kidney failure are at high risk of cardiovascular disease and death. This study uses information from 1446 people from seven European countries who have kidney failure, are receiving dialysis, have type 2 diabetes, and are prescribed either insulin identical to that made in the body (human insulin) or insulins with engineered extra features (insulin analog). After three years, fewer participants receiving analog insulins had died, had been admitted to the hospital, or had a cardiovascular event (heart attack, stroke, heart failure or peripheral vascular disease). These findings suggest that analog insulins should be further explored as treatment leading to better outcomes for people with diabetes on dialysis.

Pulsed field ablation (PFA) is an innovative technology recently adopted for treating atrial fibrillation (AF). Preclinical and clinical studies have reported a reassuring safety profile because of its tissue-specific effect, sparing adjacent tissues.1 However, 2 cases of acute kidney injury (AKI) secondary to hemolysis after PFA have recently been reported after an uncommonly high number of applications.2,3 Similarly, a subsequent pilot study reported the occurrence of AKI in 4 of 28 patients after PFA for AF.

The utility of atrioventricular (AV) optimization (AVO) algorithms remains in question. A substudy of the SMART-AV trial found that patients with prolonged interventricular delays ≥70 ms were more likely to benefit from cardiac resynchronization therapy (CRT) with AVO. The SMART-CRT trial evaluated AVO on the basis of these results, but the study was underpowered.

This paper forms part of a series of methodological guidance from the Cochrane Rapid Reviews Methods Group and addresses rapid qualitative evidence syntheses (QESs), which use modified systematic, transparent and reproducible methodsu to accelerate the synthesis of qualitative evidence when faced with resource constraints. This guidance covers the review process as it relates to synthesis of qualitative research. ‘Rapid’ or ‘resource-constrained’ QES require use of templates and targeted knowledge user involvement. Clear definition of perspectives and decisions on indirect evidence, sampling and use of existing QES help in targeting eligibility criteria. Involvement of an information specialist, especially in prioritising databases, targeting grey literature and planning supplemental searches, can prove invaluable. Use of templates and frameworks in study selection and data extraction can be accompanied by quality assurance procedures targeting areas of likely weakness. Current Cochran
